I understand that Keystone State Camps, Albright College, Slippery Rock University, and California University do not carry medical insurance for students, and I hereby certify that my child,,is covered by a personal medical insurance policy that I have in force. Further, I hereby authorize routine medical dispensary care for the above-named student, and I authorize treatment not considered routine to be referred to local physicians and medical facilities at my expense.

Fee: $395 if paid before JUNE 1. Commuter Fee: $330 if paid before JUNE 1. A $75 non-refundable deposit must accompany this application. The balance will be $320 (or $255-commuters) if received before JUNE 1 (after June 1 the balance will be $345). PERSONAL CHECKS WILL NOT BE ACCEPTED AFTER JUNE 1. Only money order, cashier's checks or certified checks will be accepted after JUNE 1. A confirmation & directions will be mailed in approximately two weeks.
